Answers to a few FAQs:
- These are in-kind services grants, there is no monetary award.
- Sites applying must be clinical practice sites providing prenatal or newborn pediatric care.
- Multi-site practices (like most FQHCs) will identify each applicant site by address in the proposal narrative (CHI considers each physical location that Centering care is provided at an individual "site").
- All practice settings are welcome to apply, you do not have to be an FQHC or CHC.
- While sites from the two focus regions will have a slightly higher proposal review score, sites from states outside the focus regions are encouraged to apply! Many of the additional considerations listed in the RFP will affect proposal scores and we do not know how many proposals will come from the focus regions.
- The Essential Elements of Centering
- Site Accreditation is included.
- A plan for other patient groups beyond CenteringPregnancy or CenteringParenting is a bonus, but one of the established models must be part of start-up. Examples of other patient groups for CenteringHealthcare: asthma, cardiac, chronic pain, HIV, hypertension, menopause, substance abuse disorder.
- There is not a PDF of the application but you can got through the various screens without entering any data to access the questions and work on them offline.
