Hello, I just finished my Annual Report and submitted it. When I go back in to "View Reports" my report does not show up there. How long does it take before this becomes available there? I just want to make sure everything was submitted correctly! Thank you!  |

Hi Stefanie, I am not seeing that the report for InterCare came through, I wonder if there is some missing piece of information (like a comment for a yellow or red flagged section)? Once it submits successfully, you should see a PDF link in the View Reports tab on the right side. It looks like the process is working fine since we got reports from a few other sites between 12/22 and 12/29. Thanks for taking care of this, I hope I can help you get it sorted ASAP!

Hi Tanya,
When I first submitted the report (or thought I did, anyway) I did see the report flash across the screen, and then it disappeared. I did have all my comments entered. I just went back in to try again, and for some reason it actually worked this time. Not sure what the issue was before. I do have the PDF file available now. Thank you!

Hi Tanya,
So now I have another question. I believe that when I initially started working with CenteringCounts online, it listed our Site Expiration date as 12/31/2018. Now it says 2019. Was this just updated? Do we submit our annual report every December no matter what? I guess I am a little confused on this whole process right now.
Also, my Annual Report did not save my survey and comments of CenteringCounts, and my graph with our health outcomes baseline/goals/actual did not include any of our baselines and some of our goals.

Hi Stefanie, it looks like your license renewal payment came in on 12/12/18 so the new expiration date is 12/31/19, thanks! The annual report due date is the same every year. I don't believe your comments save onto the PDF but they did come through just fine for us to read. Baselines, actuals and goals look good on our end, too. If you want to email me the PDF I can have a look and see if anything is missing, thank you! |

I am working on our annual submission and I noticed some differences in the dashboard compared to the patient outcomes section. I just want to make sure I am submitting everything right.

Hi Dawn,
The dashboard will always show you the last year's data from today, and the Annual Report is pulling data for your reporting period which is likely different. If you are able to submit, you've done everything correctly! Thanks for asking, we're here if you have other questions.
The Annual Report is still being worked on as we realize it is not giving the most useful information regarding groups / sustainability. But the model fidelity and outcomes sections are working correctly.
