Hello Been trying to get the annual report completed. I did get the outcomes to download but they are still not showing up in the view report. I am struggling with this part of the completing the reports.

Hi Colleen - sorry you are having trouble submitting! There is one bug that we are working on fixing: it's possible that in order for your outcomes from the dashboard to show up in the annual report you have to click the blue save button for Quality Measures on your dashboard. This confirms that these are the outcomes you've selected to follow. Let us know if that works for you! The annual report submission is still a work in progress so let us know if any other questions come up while submitting. Thank you for trying out the new system!

Hi Colleen - I do not see a report submitted from Providence yet. Are you following the 5 steps under Annual Report and then clicking Create Annual Report? Then you can review each tab (Model Fidelity, Sustainability...) and when you click Save on the Save tab it should submit to us. For anyone having trouble submitting the Annual Report, we just added this tip to the FAQ on the help.centeringcounts.org site:
1) Make sure that any red or yellow flags on the Model Fidelity and Sustainability pages have comments.
2) If you have been going in and out of the Create Annual Report page, you may need to click on each tab (Model Fidelity, Sustainability, Patient Outcomes, Cost Savings) once more and then the Save tab will be active. We hope to have a fix for this particular issue soon!

I had some trouble as well when trying to get our Annual report submitted. I ended up calling IT and having some support. In the end, I ended up switching computers as my server was not supporting this action because of an upgrade it needed....(Everything else on my computer works fine so I chose to try another computer before updating) Try that first and see if that helps. I couldn't believe I had struggled with trying to get it to submit all that time and just needed to try a different computer. Hope you have been able to get it done by now or it happens soon. Always nice when we see its done and checked off our list.
