Go to CenteringHealthcare.org and complete a Readiness Assessment, a scoring tool to help you understand where your practice falls relative to key Centering sustainability factors. You will receive an email detailing the elements of your score and suggesting next actions. You will also be assigned a CHI Practice Services Manager who will contact you to discuss any questions you may have and help you understand your options for moving forward.
